Please join Edinburgh Health and Social Care Partnership’s Long Term Conditions Programme as we celebrate the launch of the Edinburgh Self Management Network during Self Management Week from Monday 28th September – Friday 2nd October).
There will be a series of bitesize sessions with the first one : Monday 28th September (13.00 to 13:30) focused on what you can learn about the Edinburgh Self Management Network.
Bringing together practitioners who support people living with long term conditions to explore self-management. Members can connect online through the new Edinburgh Self Management Network online community of practice to:
• raise awareness of self-management services accessible to citizens through health, social care, and third sector partners
• share resources and events
• share knowledge, learning, and ideas
• benefit from peer support and networking opportunities

Tuesday 29th (13:00 to 13:45) : Measuring the difference we make: Self Management shared outcomes
Wednesday 30th (13:00 to 13:45) : Building Capacity to Deliver Lifestyle Management Courses in Edinburgh
Thursday 1st (10:00 to 11:00) : Delivery of Self-Management Support during COVID – LCIL sharing learning
Friday 2nd October (13:00 to 13:45) : Developing a Self Management Toolkit for Practitioners
