It is with great pleasure that we can publish our first report into the link working service in SW Edinburgh which EVOC is currently hosting in partnership with NHS Lothian Headroom.
Funded by Scottish Government Primary Care Division in the Health and Social Care Integration Directorate, this work has seen a successful roll-out of two link workers supporting seven GP Practices across the area.
The CAMs provided an in-depth link-working service for patients between the NHS and Third Sector services in the local community.
We’re very excited that this model of working is helping to contribute to the emerging rich landscape of initiatives to support people across the city.
